2 Proofs of Residency from list:
- Filed Homestead Exemption
- Home Mortage Document or Property Deed
- Rental/Lease Agreement (must be notarized)
- Utility Bill
- Automobile Tag Receipt
- Driver's License / State or Nat'l ID
- Voter Precinct Identification
*All 7th grade students must provide a 7th Grade Shot Record on a Form 121

- Birth Certificate
- Social Security Card (preferable)
- Immunization Record (Form 121)
- Withdrawal Papers from Previous School
- Transcripts for any student in grades 7-12
- 2 Proofs of Residency (Refer to the list under Returning Students.)
